When do babies start sleeping with a pillow?

My son is 13 months old and right now still sleeps in a play pen with no pillow. I was wondering when your supposed to have them start using a pillow? Also right now he will only sleep with a teddy bear on each side of him, a blanket over him, his pacifier in one hand and his bottle of milk in the other. How do i transition him to going to sleep with no bottle and no pacifier?
